Registration for all activities on Thursday Sept. 4 and Friday September 5 will be processed on one master form.
You may pay by credit card, check or purchase order. The University of Hawaii Conference Center is handling registration.

DEADLINES:

For conference via online - Monday Aug. 25, 2008

For conference via mail or fax - received by Friday Aug. 29, 2008

For tradeshow and poster exhibit: Online, mail or fax - Friday Aug. 15, 2008

Form of payment must accompany the registration. Credit card numbers submitted with registration will be processed upon receipt.
Registrations will not be processed without payment.

REFUNDS: Requests for refunds will be received at the UH Conference Center by Wednesday Aug. 27, 2008, in writing (email, fax or post). No refunds will be made thereafter. Please allow approximately three to five weeks for processing.

ACCESSIBILITY ASSISTANCE: If you would like assistance due to a mobility, hearing, or sight impairment, you are warmly encouraged to contact the Conference Center by Friday Aug. 8, 2008 at (808) 956-8204.
